Eligibility For Treatment

All students registered for classes at Loyola University are eligible for services provided by the department. Students may only be treated in the Student Health Service during the semester(s) for which they are enrolled in academic classes. Students entering Loyola University as a minor can be treated in Student Health Service. The parent or guardian must sign the permit to treat located on the Health History Form.

All students must complete a Health History questionnaire which establishes a personal medical record. This questionnaire is required prior to any examination or treatment given.

Faculty and staff may be treated for accidents or minor illnesses. Usually medication will not be given to a faculty or staff member, however a prescription can be written. Students are treated as the primary patient and faculty/staff may be treated should the student schedule permit.

Treatment is not provided to conference guests except on an emergency basis. Conference guests may be referred to a physician and/or hospital for treatment. Summer camp participants may be treated in Student Health Service for an accident should a parental release form to treat accompanies the patient to the department.
